We are seeking compassionate and dedicated Support Workers to provide high-quality care and assistance to individuals in various care settings, including assisted living, home health, and community care environments. The Support Worker plays a vital role in supporting residents with daily activities, health monitoring, and emotional well-being. This position offers an opportunity to make a meaningful difference in the lives of those with diverse needs, including seniors, individuals with developmental disabilities, autism, dementia, or other health conditions. The ideal candidate will possess a strong sense of empathy, excellent communication skills, and relevant experience in patient care and support services.
Responsibilities
Assist residents with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support
Monitor and record vital signs and patient observations to ensure health and safety
Administer medications following care plans and HIPAA regulations
Support residents with meal preparation and nutritional needs while adhering to dietary restrictions
Provide companionship and emotional support to promote mental well-being
Assist with mobility using equipment such as Hoyer lifts or other assistive devices
Maintain a clean and safe environment through light cleaning duties
Follow care plans meticulously, including documentation of patient progress and changes in condition
Support residents during activities and social engagement to foster community involvement
Respond promptly to emergencies with appropriate first aid or CPR when necessary
Collaborate with healthcare professionals to ensure comprehensive patient care
Qualifications
Prior experience in group home settings, assisted living, nursing homes, or home & community care preferred
Knowledge of patient monitoring, medication administration, and vital signs measurement
Certifications such as First Aid, CPR, or HIPAA compliance are highly desirable
Experience working with people with developmental disabilities, dementia (Alzheimer's care), autism spectrum disorders, or long-term care is advantageous
Ability to assist with ADLs and provide compassionate patient-centered support
Physical ability for heavy lifting and assisting residents with mobility needs
Strong communication skills and patience when working with diverse populations
Relevant experience in hospice care, home health, social work, or caregiving is a plus
