Pay Rates: 14.89 19.36

Shift Patterns: 37.5 hours per week (7.5 hrs per day) 7:30 - 15:30 or 8:30 - 16:30 Can be flexible with part-time hours if required

Stroke Early Supported Discharge Team Associate Practitioner will be responsible for Assisting in identifying health and social needs of patients and participate in the delivery and evaluation of care provided under the indirect supervision of qualified staff (registered Nurses, Physiotherapists, Occupational Therapists and Speech and Language Therapists), Ensuring high standards are maintained in a safe environment, which promotes equality and sensitivity for all individuals.

Key Activities / Responsibilities

oAs a member of the interdisciplinary team covering both the inpatient stroke unit and community, the post holder will be trained to take responsibility for carrying out delegated, relatively routine assessments spanning the professional domains of nursing, physiotherapy, occupational therapy and speech and language therapy. The results of which will be fed back to registered health professionals to establish subsequent care/activity plans.

oThe post holder will take responsibility for delivering rehabilitation programmes, delegated by professionally qualified staff, which can be performed safely with training and under protocol and direct or indirect supervision.

oThe post holder will monitor the outcomes of the rehabilitation provided, to make recommendations to minor changes and refer on to professional qualified staff as appropriate.

oThe post holder will document all aspects of care in which they have been directly involved. This will include assessments, observations and care provided.

oTo undertake trusted assessor training and applied responsibilities.

oTo complete 6 month stroke reviews as per the Stroke Pathway under the direction of the Stroke Nurses.

oTo contribute to the development of effective and efficient team work in the delivery of care.

oTo ensure that professional and own behaviour is managed in line with Trust competencies.

oMaintaining clinical records on systems including e-carelogic and patient centre. To be independent in the use of the internet and email and excel.

What you'll need:

oSignificant experience of working in a relevant health or social care setting essential Experience of working within stroke care desirable Experience of working in the community desirable

oQualifications A-Level qualifications or equivalent (e.g. NVQ 3); Health & Social Care NVQ level III or above experience or Care certificate completion within the first 6-12 months of employment if new to the Trust English and Maths GCE/GCSE or equivalent Grade C or above or equivalent qualification to demonstrate numeracy and literacy

oKnowledge of nursing and / or physiotherapy / occupational therapy / speech and language therapy procedures and practices.

oFull driving license

oPhysically fit and able to comply with trust manual handling guidelines Health and Social Care Foundation Degree or equivalent.

oKnowledge of stroke rehabilitation IT skills
