An exciting opportunity has arisen for Administrators to work within the Somerset Hub for Coordinating Care team. The induction period for this post will be based at Priory House in Wells, with a view to be relocated to Ashford Court in Taunton, once training is completed.
The Somerset Hub for Coordinating Care consists of the Intermediate Care Bed Hub, Somerset Primary Link Urgent Referral Service, Hospital at Home Remote Monitoring Service and End of Life Care Coordination Centre. We are a friendly, hard working team and we heavily focus on health and wellbeing.
This job role will predominantly be based within the Hospital at Home Remote Monitoring Service but successful applicants will have the opportunity to work across the teams and support during staffing pressures.
You'll have strong attention to detail, advanced multitasking skills and will be comfortable working within a busy working environment.
Our service hours are 08.00 AM to 08.30 PM, 7 days a week, every day of the year and our service does not close. We are also open on all bank holidays.
Main duties, tasks & skills required

You'll work closely with the Somerset Hub for Coordinating Care clinicians and administrators in facilitating a smooth referral process for service users and patients, providing a single point of contact for information on patients to be transferred / admitted to community hospital beds within the Primary Care Trust. You will be required to take Deep Vein Thrombosis and Rapid Response Therapy Referrals, escalate unwell patients in the Community to Clinical Staff or Management.
You may be required to support the Somerset Hub for Coordinating Care clinicians in providing an efficient referral process, to include booking transport and liaising with patients, as well as your own workload.
You will be required to take on supervision of the Patient Transfer Facilitators and liaise closely with the Operational Administration Managers.
You will be required to work across multiple computer system's, coordinate telephone calls and completing a range of reports.
Participate in team and Trust meetings as requested. You will be required to take minutes or type up minutes from recorded meetings.
