Are you passionate about community care and leading a dedicated team? We're seeking a dynamic leader at Rochdale Infirmary to support residents of Heywood, Middleton, and Rochdale (HMR).



As a Team Lead, you'll manage staff, oversee rotas, recruitment, and supervise learners. You'll monitor sickness absence, annual leave, and training, ensuring compliance with trust policies. You'll play a key role in delivering the Hospital@Home service, ensuring timely, appropriate care is provided in patients' own homes.



You'll work closely with patients and carers to manage health needs, promote self-care, and develop personalized plans. Collaborating with multidisciplinary teams--including NWAS, GPs, pharmacy, and the wider Neighbourhood Team--you'll help shape high-quality, patient-centered services aligned with trust goals.
