Job Summary:





The Team Leader is an integral part of the support team, responsible for meeting the practical, physical, and emotional needs of the individuals we support within the service.


Through a key worker system, they will design individual support programmes and coordinate the delivery of these packages by a group of Support Workers and themselves.

Candidate note: This role is located on the Isle of Man and applicants must either be a resident or hold a work permit at time of appointment to take up the role



About The Role



Key Accountabilities



Plan, coordinate, and implement the safe transition of new individuals into the service and community as needed. Communicate with the families of the individuals we support and relevant members of multi-disciplinary teams, community groups, and voluntary bodies to advocate for their needs. Plan, develop, and implement individual and group support programmes to enhance and maximize the capabilities and independence of individuals in the service. Coordinate regular assessments and reviews using appropriate assessment models. Address the physical, emotional, and personal needs of individuals, ensuring that work is focused on individual needs and upholds principles of respect, privacy, dignity, fulfilment, independence, and choice. Ensure that medication is held, stored, and administered according to Praxis Care's Medication Policy and current statutory and legislative procedures. Provide effective leadership, supervision, and management to Support Workers, including monitoring time and attendance and participating in capability, disciplinary, and grievance procedures as necessary. Participate in organizational learning and development opportunities while identifying personal development opportunities for themselves and Support Workers. Ensure that adequate records are created and maintained and that necessary reports are produced in compliance with organizational policy and statutory requirements. Assist the manager with compliance concerning all relevant regulatory bodies.

Requirements of the Role



Participate in the company appraisal and staff review process. Adhere to and model the company values, behaviours, and competencies at all times. Engage in formal and informal supervision as required. Carry out all duties in accordance with Praxis expectations, regulations, and policies. Be prepared to take on additional duties as necessary. Work the necessary hours to meet the needs of the service.

The above-mentioned duties are not exhaustive, and the post holder may be required to carry out other duties as necessary. It should be noted that stated duties or locations associated with the post may change to meet the future needs of the organisation.

Essential and Desirable Criteria:



Education, Experience & Training Qualifications





Essential:

2 years' experience in a health and social care setting and currently enrolled in QCF Level 3 in Health and Social Care, OR Understanding of the mental health/learning disability area, AND Qualification in: + QCF/RCF Level 3 in Health and Social Care
+ Social Work
+ First Level Nursing
+ Allied Health Profession
+ Level 5 Diploma in Leadership for Health and Social Care (adults management/adults residential management, Wales & NI).


Desirable:

Experience in mental health or learning disability services.

Skills & Competencies



Full valid driving licence and access to own vehicle (if a disability precludes an individual from holding a driver's licence, it should be stated how they can meet the mobility requirement of the post). Strong verbal and written communication skills. Report writing skills. Assessment and evaluation skills. Computer skills. Risk assessment skills. Leadership skills. Problem-solving skills. Required Criteria



2 years of experience in a health and social care setting Full valid driving licence and access to own vehicle Will undertake a professional qualification in: Social Work, 1st level Nursing, Allied Health Profession, Level 5 in Leadership and Management for Health and Social Care Right to Work in the UK

Desired Criteria



Experience in leadership or management roles within health and social care.

Skills Needed




Supervision, Communication, Teamwork, Ability to support clients with learning disabilities and/or complex needs
