To assist in the provision of care and support and work as part of a team to achieve required standards.

To provide care and support to vulnerable adults in their own homes to promote and work within the

professional code of practice to provide a high standard service. Promoting and encouraging

independence, dignity and individuality. Providing a service which is person centred.

We are seeking a dedicated, compassionate and experienced Team Leader to join our dynamic team and support our mission to provide exceptional care in the community. The team leader role will play a crucial role in ensuring the individuals who use our service receive the highest quality, person centred service within their own homes

This role involves direct supervision for all care staff, offering support and assistance where necessary.

JOB DESCRIPTION:



As a team leader in domiciliary care, you will play a crucial role in ensuring the effective delivery of care services to the individuals who use our service.

You will lead, support and motivate a team of care workers to provide high standards of care, promoting independence and enhancing the well-being of our individuals.

Your responsibilities will include overseeing care plans and risk assessments, ensuring they are accurate and fit for purpose, updating the leadership team of any changes and updates that need to be completed.

Supervising staff and liaising with individuals, family members and other stakeholders to ensure a seamless and high-quality experience.

Supporting the staffing team within the community to ensure they are aware and following all company policies and procedures, and that all staff are working in line with the code of practice and company handbook.

Helping and supporting staff to upskill and develop their knowledge and working practice around task specific aspects of individuals care and support packages.

Completing competency checks on the staffing team and liaising any actions or cause of concerns to the management team.

Completing audits and paperwork inline with your job role.

KEY RESPONSIBILITIES:



Assessing individuals needs and participating in supporting the care coordinator and managers in creating/developing personalised, person-centred care plans and risk assessments. Supervise, support and motivate a team of care workers to deliver high quality care and support. Assist and provide training and development for the staffing team. Conduct spot checks every month for each member of staff and audits to monitor the quality of care being delivered. Any actions or cause of concerns must be communicated effectively with the management team. Implement and monitor action plans to address any areas for improvement. Ensuring compliance with care standards and regulations. Liaising with individuals, their family members and any other relevant professionals involved within the individuals care package. Monitoring and reviewing care plans to ensure they are fit for purpose, person centred, effective and that it meets the needs of the individuals. Monitoring the individuals care package and updating paperwork and the relevant professionals when there are changes/updates. Ensure timely and effective delivery of care services. Support and supervise care staff, ensuring they provide the highest quality service. Maintain accurate and up to date journals / records and documentation for individuals and care staff. Act as a key point of contact for individuals, families and other health professionals. Coordinate with other services and agencies to ensure holistic care for individuals. Maintain accurate and up to date records and documentation. Holding the on call phone, mainly during allocated weekend shifts. Answering the phone and dealing with staffing queries, supporting the staffing team, proving the staff with advice, dealing with emergencies, communicating effectively with individuals who use the service. Participate in the on-call rota to provide support out of hours as needed.
Communication;

- Act as a primary point of contact for the individual who use the service and their family members, and health care professionals.

- Communicate effectively with all stakeholders to ensure seamless care delivery.

- Effective and clear communication with management team to ensure all updates and changes are communicated.

Compliance;

- Ensure all care services comply with relevant regulations and legislation with the support from management team.

- Keep up to date with industry standards and best practices.

- Be aware of all policies and procedures and keep up to date with any changes / updates and embed these within the staffing teams.

QUALIFICATIONS:



Must have QCF Level 2 & 3 in health and social care or working towards this qualification. All company mandatory training courses must be complete and in date. Strong organisational and time management skills. Experience in a supervisory or leadership role within domiciliary care. Understanding of regulatory requirements and quality standards within domiciliary care. Excellent communication and interpersonal skills. Ability to work independently and as part of a team. Ability to supervise and motivate a staffing team. Valid driving licence and access to a reliable vehicle.

This role required occasional evening work and commitment to working every other weekend. This role also has the requirements of travelling around the community to individuals' homes.

This job role also requires the commitment to holding the on call work phone out of working hours. The on call work phone is to provide support and assistance to care staff and individuals who use the service and deal with any emergencies.

Job Types: Full-time, Part-time

Pay: From 12.80 per hour

Expected hours: No less than 22 per week
