Purpose of the role of the Support Workers



To be part of a team providing a high-quality service for people with learning disabilities/mental health/autism, which enables users to live with a maximum of choice and independence, participating in a wide range of community based activities, designed to meet their individual needs. Using a person-centred approach when assist people with learning disabilities/mental health/autism, and help them to live more fulfilling lives. Develop and use their individual skills via the provision of support, assistance and personal care. The post holder will be required to work shifts including weekends, bank holidays on a rota shift system as directed.

Main duties of the Support Workers



To assist in providing practical and personal support to enable service users to have more control over their lives and to meet their assessed needs by maintaining and promoting their health, wellbeing and social development. To ensure service users are supported in a varied range of activities which promote the development of people as individuals, increase independence and maximise community inclusion as identified both through day to day choice and their agreed care plan. Support service users to make informed choices and have new experiences within an agreed plan of risk taking. Assist the provision of the highest possible standards of support with regard to quality of life and a homely environment including support for those users who represent a challenge to the service. Support where necessary service users to attend social and leisure activities, community outings and appointments. To assist service users with their support needs as appropriate, ensuring awareness of each individual's culture, dignity and showing respect at all times. This may involve prompting to do key activities of daily living such as washing, dressing, feeding, or administering medication as required. To assist service users in meeting outcomes from their person-centred plan and care plan. Assist service users in cooking, shopping, cleaning, gardening, laundry, ironing and household tasks. Participate in achieving the highest domestic standards with respect to cleaning, hygiene and provision of meals for well-balanced diets that meet the cultural and dietary needs of the residents. To support Service Users to monitor their health and wellbeing Take responsible care of service users' property, money and other personal effects and maintain appropriate records. Report all incidents and accidents involving residents to Team Leader/ Deputy/Service Manager. To complete the Service Users` support notes at the end of each shift. To maintain complete and accurate records including Service Users personal files and financial records. To provide regular oral and written records & reports to the Supported Living Manager on the progress of individual Service Users. Support Service Users to document all appointments in their diaries and to inform the Manager as appropriate. Work in the framework of a staff rota, based around the residents' needs and the requirements of the organisation. To attend and participate in supervision and appraisal meetings with line manager. To immediately report any alleged abuse through the vulnerable adults' procedure. To work in partnership with Service Users, agencies, families and stakeholders. To use the out of hours on-call support service and emergency services as appropriate. Attend training programmes as directed and seek to continue learning and development in the changing national and local policy context. The post holder must at all times carry out his/her duties with regard to CQC National Minimum Standards for Care Homes, company's Customer Care and Equal Opportunities Polices and in accordance with company's Mission Statement and Values. To undertake any other relevant duties of a similar nature and level as required.

Requirements of the Support Workers



This post is service based but will involve some out of office activity and travel across to other sites. A positive attitude, friendly personal and a caring nature.
Must be able be flexible with their work hours.
