Job Description

This is an exciting opportunity to join the Connected Families Service as part of the Connected Coach team who work alongside Children's Social Care with young people aged 11+ who are identified as being on the edge of care.



The Connected Coach team has had a significant impact on the lives of young people referred over the last 2 years, with a high percentage of those receiving support successfully remaining within their family networks. The Service is a key part of East Sussex County Councils strategy for supporting children and families, and reducing the number of young people entering into the care system.



The multidisciplinary team has a focus on provide intensive support to reduce risk and stabilise young people's home environment so they can remain living within their family network. The team includes clinical staff working closely with Connected Coaches' whose main role is to deliver supportive interventions. The multi-disciplinary team support in the formulation of need and supporting or delivering direct interventions for children and their families.



As a team lead you would hold a small case load of complex young people and or their parents/carers. You would also have a line management and supervisory role for a proportion of the Connected Coach team members.



You will lead and manage a team of professionals in the multidisciplinary Safeguarding Connected Families service where you will be working with parents/ carers, young people and their families. You will manage a small team of Connected Coaches and hold a small case load working with Adolescents age 11-17 who are at risk of coming into Local Authority Care.



SWIFT Specialist Family Services (SWIFT) is a well-established integrated multi-agency service between ESCC Childrens Services and SPFT.



SWIFT provides assessments and interventions to families involved with ESCC child protection and safeguarding services, including those in court proceedings. Particularly when the parent(s) own difficulties are impacting on their ability to parent. For example, substance misuse, mental health, sexual risk, domestic abuse, and learning disability.



The service is led by the local authority with a multi-disciplinary management team and consists of practitioners from a variety of backgrounds, including social workers, psychologists, psychiatrists, mental health nurses and family workers.



This post is part of the Connected Families Clinical MDT linked to SWIFT CAT, which offers assessments and interventions to children who live in these complex high-risk families. This is a multidisciplinary child assessment and treatment team contributing to the mental health service offered to children by SWIFT.



You will hold a current registration for your qualification and 5 years minimum post registration experience. You will have sound experience of undertaking assessments focusing on complex adolescent presentation / Contextual Safe Guarding / Child Exploitation, Neuro Diverse presentations and delivering interventions to support young people with these concerns and risks who are at risk of coming into Local Authority Care. You will hold a small, complex clinical caseload within the designated care group where you will provide and oversee a high quality clinical service to young people/ adolescents and their families assessing and delivering interventions/ consultations to Locality Social workers working in conjunction with the Clinical MD specialist team.



You will provide and oversee a safeguarding response to Children's Services to cases in need of urgent clinical support and manage incoming referrals from Children's Services.



You will contribute to the achievement of high quality standards of care and deliver/effective leadership within a multi-disciplinary clinical team. You will supervise and manage junior staff/support the Clinical Lead Nurse Specialist (Child) and Operations Manager (Children) with ensuring clinical governance and evidence based practice is in place.
