Radis Community Care have an opportunity for a Team Leader to join our team based in Clarence Park Extra Care in Malvern


About the role


As a Team Leader, you will be the first point of contact for the people we support and staff onsite as well as visiting health professionals. This can include:

Leading the care shift and dealing with any emergencies that arise on shift Creating/updating Care Plans, Risk Assessments and other key documentation Auditing documentation such as MAR Charts, Daily Diary Records etc Completing staff rotas to ensure each shift is adequately staffed Completing quality monitoring checks such as spot checks, carers assessments & reviews Liaising with other Health Professions such as Doctors, District Nurses etc.

About You

Be able to work a mix of early shifts, late shifts, and alternate weekends Be willing to take part in a on call rota Worked in the Health & Social Care sector for a minimum of 2 years Be a caring individual who is passionate about putting the client first and ensuring we provide an Outstanding service we would want our own families to receive.
