Job Description

This post holder will join a small care team based in Northampton, supporting A, a 16-year-old girl who sustained a brain injury and has a learning disability. A presents with the mental capacity of a 3-year-old and currently lives at home with her family. She is continuing her rehabilitation journey with the support of a dedicated team of three support workers and ongoing therapy input.

The employer for this post is the Deputy acting on behalf of A.

15 -24 hours

Experience in care and support of children/young people with a disability Experience in brain injury/learning disability Experience of working with therapists in a rehabilitation setting Experience of community access Experience liaising with families and the case management team Computer-literate and contactable by e-mail Ability to maintain clear and concise records Excellent communication skills Flexibility in approach to care Ability to act on own initiative Great sense of humour and patience Clean driving license Flexibility to travel with the client Experience in care and support of children/young people with disabilities Being on the DBS update service is desirble

Qualifications



Willingness to undertake further training NVQ level 2,3, or 4 Health & Social Care Children & Young People (Desirable)

Direct work with Client



To provide support during personal care tasks. Supporting A to attend school and therapy appointments. Implement and monitor therapy guidelines provided by the treating team. Monitor diary arrangements and support A in organisation and structure. Maintain the home in accordance with A's needs (including cooking, cleaning, shopping). Support and encourage activities to assist with ongoing rehabilitation and learning. Support ongoing relationships with family members and friends. Promoting purposeful and safe leisure time, both at home and in the community. Monitor all health care needs specific to A. Assist with planning and organisation of daily tasks and non-routine activities. Provide companionship and reassurance, building confidence and self-esteem. Ensure documentation and monitoring sheets are correctly recorded and filed. Ensure the communication book and daily record are completed. Oversee and ensure A's well-being, safety, and happiness. Participate in the smooth running of the household, completing domestic chores as required. Empower A in all aspects of her life. Liaise with the Case Manager about general progress and any incidents or concerns Monitor support worker float and maintain accurate financial records.

Team Leader - Additional responsibilities



To manage the client rota, including staff leave. To support new staff during their probationary period e.g. shadowing To ensure staff training and safeguarding competencies are up to date with the support of the support manager Ensure care plan, therapy guidelines, and risk assessments are up to date and reviewed regularly with the support of the case management team To respond to and manage any incidents and refer to the case manager when required. To coordinate and support the staff team daily. To oversee the client float and ensure records are sent to the case management team at the end of each month. To disseminate any changes promptly and accurately in clinical care delivery to the rest of the team. To monitor all clinical care needs and liaise with the treating team and case manager
