Job Description

Overview:

The London Borough of Harrow is seeking a dedicated and experienced Team Manager to lead our innovative, newly established Adults Integrated Front Door Service.

You will be central to a new service model focused on preventing, reducing, and delaying demand for long-term care. This is achieved by effectively triaging and resolving resident needs through successive tiers of preventative interventions, thereby reducing pressure on our long-term case management teams. A key component of this model is the integration of Care Management and Occupational Therapy resources and practice to ensure a more holistic and joined-up service.

As Team Manager, you will be a strong leader responsible for overseeing a multi-disciplinary team, which includes Social Workers, Care Managers, Occupational Therapists, Occupational Therapy Assistants, and administrative staff. Your core focus will be to ensure the provision of high-quality care and support for vulnerable adults.
The successful candidate will use their strong leadership skills to manage complex cases and promote best practice across the team, ensuring full compliance with relevant legislation.

Key Responsibilities:

Manage, develop, & support a team of Social Work and OT practitioners. Provide guidance, reflective supervision, and manage performance issues. Oversee the delivery of high-quality, person-centred services that meet statutory guidelines and organisational standards. This includes making evidence-based decisions and applying a strengths-based approach. Compliance and quality assurance: ensure the team meets legal obligations, such as those under the Care Act, and upholds standards for case recording. Manage the team's staffing budget and any delegated budgets for care packages, ensuring financial policies and procedures are followed. Work with local partners, other agencies, and service providers to ensure effective service integration and communication. Support the professional and personal development of team members. Manage complex and sensitive situations, with a focus on safeguarding and risk management. Foster a positive and professional team environment. Maintain accurate records and contribute to service development.
