About the Role:
We are looking for a motivated and skilled Integrated Team Manager to join our Isle of Wight Integrated Locality Team in a full-time position.
As our Localities Team Manager, you will oversee the Locality Multi-Disciplinary Teams, ensuring that those in need of adult social care receive safe, timely, and person-centred support. This role is all about leadership, collaboration, and innovation in service delivery.
What You'll Be Doing:
Leading the delivery of high-quality social care assessments and practice in line with the Care Act.
Championing a person-centred, strengths-based approach, placing individuals at the heart of every decision.
Encouraging independence to minimize the need for formal care whenever possible.
Continuously improving service efficiency through collaboration with key stakeholders.
To be successful in this role, you'll need to:
Hold a Social Work qualification and be SWE registered
Be an accredited Practice Educator or AMHP
Thrive in a fast-paced environment
Have experience chairing meetings
Demonstrate expertise in person-centred and strengths-based approaches
Possess strong knowledge of adult social care legislation and safeguarding
Be confident in decision-making and service improvement
Understand how data supports teams and service delivery
Lead with passion and drive, promoting strengths-based care
Have proven experience leading and working within a team
Collaborate effectively with partner agencies
Be prepared to take part in the Out of Hours Stand-by Duty Rota as required
