An exciting development opportunity has arisen for a Team Manager to join the countywide Transforming Lives Team. We are looking for someone who is passionate about enabling people to live fulfilling, meaningful lives and who can inspire others through a compassionate, strengths?based model of leadership.

About the Team




The Transforming Lives Team is rooted in creativity, collaboration and a commitment to supporting adults to thrive. The remit includes:

Championing positive transitions for adults with significant care and support needs who are discharged from hospital under the Learning Disability and Autism Programme. Working in partnership with the Community Forensic Intellectual Developmental and Disability (CFIDD) service -- a skilled, multi?disciplinary team supporting people with multiple needs and high-risk presentations. Social Workers contribute through Social Supervision, offering high?quality assessments and reporting to the Ministry of Justice for individuals subject to Section 41 of the Mental Health Act. Completing Care Act assessments for adults residing in HM Prisons across Nottinghamshire, ensuring their strengths and needs are clearly understood and that support is coordinated to enhance wellbeing. Building and sustaining positive partnerships with Health colleagues, statutory agencies, providers and the voluntary sector to create the conditions for people to achieve better outcomes.

The team is made up of experienced and passionate practitioners -- including a Team Manager, Advanced Practitioners, Social Workers, Occupational Therapists and Community Care Officers -- all working within a hybrid model based at Sherwood Energy Village.

Demonstrates a strong understanding of legislation and policy relating to mental health and uses this confidently to support person?centred practice. Is committed to the departments Local Account and to modelling a person centred approach in all aspects of leadership. Works collaboratively across systems, enabling effective joined?up support that promotes independence and positive risk?taking.

This role is ideal for an experienced manager who is eager to grow their leadership skills in a dynamic and specialist service area.

Essential Experience and Knowledge




You will be:

Registered with Social Work England. Experienced in working with adults facing complex circumstances, drawing on strengths?based practice and high?quality professional judgement. Skilled in applying the Mental Health Act (1983) and Mental Capacity Act (2005) to real?world situations with confidence and clarity.
