Working Full time:

37.5 hours per week



This is an exciting opportunity to work as a Band 3 Therapy Assistant within our Hospital at Home Team at the Countess of Chester Hospital NHS Foundation Trust.



We are looking for a motivated, enthusiastic, and dynamic Therapy Assistant to join our supportive team.



An individual who can integrate quickly into a team setting and will support in driving service provisions.



Good communication, time management, team working skills and the ability to work under pressure, are essential requirements to this post, as well as experience of working with adults in a care setting.



The post will involve working with patients, in the place they call home, during an acute physical or medical crisis. The role will involve supporting therapists in the management of individuals following a fall, providing therapy and care interventions and supporting the wider multidisciplinary team with tasks to support our patients to ensure we achieve Safe, Kind and Effective Care.



To support and assist therapists/nurses to carryout treatment plans and implement care plans to a designated caseload.



Provide a high quality, effective therapy service to patients within their own home.



Provide treatment interventions to patients with diverse clinical presentation as indicated by a qualified therapist.



Feedback therapy and care interventions and concerns to a qualified therapist or nurse.



The Countess of Chester Hospital NHS Foundation Trust provides services to West Cheshire and to Welsh patients covered by Betsi Cadwaladr University Health Board. The Trust works collaboratively within the wider Cheshire and Merseyside Health and Care Partnership. Its services are provided from three locations:



The Countess of Chester Hospital:

providing 438 general and acute beds



Ellesmere Port Hospital:

providing 56 beds as a rehabilitation, intermediate and outpatient facility



Tarporley War Memorial Hospital:

a base for community services which serve the local rural population.



The Trust employs over 5916 staff (headcount) which includes temporary bank staff and provides acute emergency and elective services, primary care direct access services and obstetric services to a population of approximately 411,000. This includes 361,000 residents in Chester and West Cheshire which includes Ellesmere Port and Neston as well as the Deeside area of Flintshire which has a population of approximately 50,000.??
