Job Description

An exciting opportunity has arisen within our dynamic team to work with people who have experienced Stroke or Brain injury as they return home from hospital and rebuild their lives. The successful applicant will work with a range of professional colleagues including occupational therapists, speech therapists, psychologists, a dietitian, and physiotherapists to support individual rehabilitation programmes with people in their own homes and communities. There will be opportunities to support the delivery of our Niwrostwt Neurological Recovery College programme and online groups. Applicants should have a level 3 qualification in a relevant field and will be required to undertake the AGORED level 4 Therapy Assistant Practitioner practice-based Qualification.



English and/or Welsh speakers are equally welcome to apply.



UK driving licence is required for this role.



To contribute to the provision of health care to the people of Gwent by providing high quality, effective support and rehabilitation services to people who have had a stroke or brain injury enabling people to return home from hospital early and rebuild meaningful lives. This activity supports the ABUHB Clinical Futures work reducing length of hospital stay, providing care closer to home and establishing home as the hub.



The post holder will carry a delegated caseload of people with stroke and other neurological conditions and will work closely with registered clinicians within the service.

You will work as part of the CNRS multi-disciplinary team and have responsibility for delegated aspects of the clinical management of people who have had a stroke or brain injury and are deemed fit to return home on the stroke early supported discharge pathway or following a brain injury under the supervision of CNRS clinicians. This may be undertaken in individual settings carrying out treatment programs independent of the therapist according to an agreed program set by the therapist. You will gather person related background information related to the early supported discharge of people who have had a stroke or brain injury from inpatient hyper acute, acute or rehabilitation settings or in the community. This may involve nursing staff, medical notes, patient interviews, liaison with family/carers or other organisations.
