This role is offered on a permanent basis. There is more than one post available with a combined total of 52.5 hours per week.



An exciting opportunity has arisen to join the Community Neurology Service, which is a countywide team created to serve patients across the region with neurological conditions via a multi-disciplinary approach. You will be a key member of the team, providing neurological rehab support alongside Physiotherapists, Occupational Therapists, Speech and Language Therapists and Psychologists.



This role is countywide and will require post-holders to have a full car driving license and access to a vehicle for business purposes. Travel is countywide, primarily in and around people's homes delivering rehab as well as supporting classes and clinics.



The post-holder will join a team of therapy support workers who cover the county and should be prepared to cover North and South Cotswolds, Stroud and the Forest of Dean.



This role is ideal for anyone with experience as a physiotherapy or occupational therapy assistant or reablement worker who is keen to progress their knowledge to the wider multidisciplinary team and be part of a innovative service.



To carry out short term rehabilitation programs and activities of daily living with individuals and carers as planned by the multidisciplinary team. This may include the completion of therapeutic personal care in order to achieve or maintain independence.



To monitor progress and function of individuals in order to contribute to the ongoing assessment of their needs by liaising with team colleagues.



To support the education of service users and their families/carers to promote self-management of their condition .



To support the organisation and facilitation of group sessions .



To inform service users of other support services and introduce them where agreed as part of a rehabilitation care plan.

The role requires countywide travel to deliver delegated rehab programmes to patients with neurological conditions.



The post-holder will deliver treatment plans, monitor progression and feedback to the multi-disciplinary team.



There will be lone working and joint working in the role, with different members of the team.



As a Health Care Assistant/ Support Worker you will be responsible for participating and assisting in the delivery of care / services to patients and service users as part of a Mental Health and Social Care Health Service. Health Care Assistant, Social Care Workers and other Healthcare Support Workers employed by the Trust are required to follow the guidance set out in Code of Conduct for Health Care Assistants/ Health Care Support Workers to provide safe and compassionate care of a high standard, and to challenge others who are not.

You will receive direct supervision and partake in reflective practice / training on a regular basis and be required to meet the Care Certificate standards. All successful applicants will also have the opportunity to discuss the prospect of undertaking a Level 3 Healthcare Support Worker Apprenticeship alongside their substantive post. This apprenticeship would include the Level 3 Diploma in Clinical Healthcare.
