We are seeking a dedicated and experienced Nurse to join our Tissue Viability team. The ideal candidate will provide advanced nursing care to patients with complex wounds, ensuring high standards of clinical practice and patient care. This role involves working collaboratively with a multidisciplinary team to develop and implement individualised care plans, using evidence-based practices to promote optimal healing and patient outcomes.

Assess, plan, implement, and evaluate individualised care plans for patients with complex wounds.
Perform advanced wound care procedures, including debridement (after appropriate training), dressing changes, and application of specialised wound care products.
Educate patients and their families on wound care management, prevention of complications, and lifestyle modifications to support healing.
Collaborate with physicians, surgeons, and other healthcare professionals to ensure comprehensive, patient-centred care.
Monitor and document patient progress, adjusting care plans as necessary to achieve desired outcomes.
Participate in multidisciplinary team meetings and case conferences to discuss patient care and treatment strategies.
Stay up to date with the latest research and advancements in wound care, incorporating new knowledge into clinical practice.
Engage in local quality improvement and audit initiatives and assist with the planning of local clinical governance meetings.
Provide mentorship and training to junior nursing staff and students.
The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
