Job Description

Clinical Responsibilities The post holder will take a lead role in the advanced clinical assessment, investigation, diagnosis, treatment and /or referral of a diverse and undifferentiated and undiagnosed problems, in accordance with professional standards and codes of conduct, national best practice and locally and nationally agreed standards. Will act as a credible expert, leading a team in the care of patients, who may have highly complex and or/ chronic presentation, developing, evidencing high-level assessment, assimilation of information implementing and evaluating personalised clinical management plans. Undertaking appropriate investigations to formulate a diagnosis implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis, complex needs or long term conditions. Prescribing within the boundaries of extended non-medical prescribing (AHPs demonstrate advanced knowledge, alongside the use of patient group directives and undertake advanced study as available).



The post holder will provide critical thinking and analytical skills in practice, and attainment of knowledge, and use their experience to support practitioners in the development of their own skills and knowledge. To act as a role model, resource and mentor, promoting excellence in practice, maintaining and monitoring standards of care, highlighting and working with others as appropriate to manage performance where there is cause for concern. Both providing and participate in clinical supervision and undertaking peer review of practice. Support the collation of the training needs analysis, promoting learning and creating a positive learning environment.



Participate in quality improvement, supports the development of service improvements, and also utilises knowledge of clinical governance to provide safe and effective services to individuals. Provides support, advice, guidance and training to patients, families and carers, to promote the understanding and consent to care. Access's research and interprets research, identifies gaps in practice, utilises appropriate evidence bases to support practice development and applies research evidence into working practice. Administrative and professional responsibilities Ensure accurate notes of all consultations and treatments are recorded in the patients notes on the clinical computer system as appropriate Ensure accurate completion of all necessary documentation associated with patient health care and registration with the Practice Ensure collection and maintenance of statistical information required for regular and ad hoc reports Attend and participate in Practice meetings as required Assist in the formulation of Practice philosophy, strategy and policy and develop appropriate protocols Lead on clinical portfolio areas Research projects Co-operate and participate as required in any research and audit projects within the Practice Student/staff education and training Participate in the education and training of students of all disciplines and the induction of all members of Practice staff Liaison Maintain effective liaison with other agencies and staff concerned with patient care and with all other disciplines within the Practice, with appropriate regard to confidentiality Professional development Attend and complete an ACP apprenticeship pathway Maintain continued education by attendance at courses and study days as deemed useful or necessary for professional development Agree and complete personal development plans through appraisal processes Confidentiality In the course of seeking treatment, patients entrust us with, or allow us to gather, sensitive information in relation to their health and other matters.

They may also have access to information relating to the Practice as a business organisation. All such information from any source is to be regarded as strictly confidential Information relating to patients, carers, colleagues, other healthcare workers or the business of the Practice may only be divulged to authorised persons in accordance with the Practice policies and procedures relating to confidentiality and the protection of personal and sensitive data Health & Safety To ensure that safe systems of work are practiced and that the agreed health and safety policies are carried out. To be aware of the responsibility of every employee to have regard for safety for him or herself and others at work in the health and safety at work act Using personal security systems within the workplace according to Practice guidelines Identifying the risks involved in work activities and undertaking such activities in a way that manages those risks Making effective use of training to update knowledge and skills Using appropriate infection control procedures, maintaining work areas in a tidy and safe way and free from hazards Actively reporting of health and safety hazards and infection hazards immediately when recognised Keeping own work areas and general / patient areas generally clean, assisting in the maintenance of general standards of cleanliness consistent with the scope of the job holders role Undertaking periodic infection control training (minimum annually) Reporting potential risks identified Equality and Diversity The post-holder will support the equality, diversity and rights of patients, carers and colleagues, to include: Acting in a way that recognizes the importance of peoples rights, interpreting them in a way that is consistent with Practice procedures and policies, and current legislation Respecting the privacy, dignity, needs and beliefs of patients, carers and colleagues Behaving in a manner which is welcoming to and of the individual, is non-judgmental and respects their circumstances, feelings priorities and rights Personal/Professional Development The post-holder will participate in any training programme implemented by the Practice as part of this employment, such training to include: Participation in an annual individual performance review, including taking responsibility for maintaining a record of own personal and/or professional development Taking responsibility for own development, learning and performance and demonstrating skills and activities to others who are undertaking similar work Quality The post-holder will strive to maintain quality within the Practice, and will: Alert other team members to issues of quality and risk Assess own performance and take accountability for own actions, either directly or under supervision Contribute to the effectiveness of the team by reflecting on own and team activities and making suggestions on ways to improve and enhance the teams performance Work effectively with individuals in other agencies to meet patients needs Effectively manage own time, workload and resources Communication The post-holder should recognize the importance of effective communication within the team and will strive to: Communicate effectively with other team members Communicate effectively with patients and carers Recognize peoples needs for alternative methods of communication and respond accordingly Contribution to the Implementation of Services The post-holder will: Apply and work within the guidelines and policies of the practice Discuss with other members of the team how the policies, standards and guidelines will affect own work Participate in audit where appropriate Any other delegated duties considered appropriate to the post
