Exciting Opportunity:

Trainee Advanced Clinical Practitioner - Integrated Specialist Palliative Care Team



We are delighted to offer an exciting opportunity for a motivated and compassionate nurse to join our team as a Trainee Advanced Clinical Practitioner (ACP) within our Integrated Specialist Palliative Care Team.



Our team of Palliative Care ACPs are expert practitioners who work across inpatient, outpatient, and community settings, including our innovative Palliative Care Virtual Ward. This role will include supporting home visits to provide high-quality care to patients in their own homes.



ACP team members act as clinical advisors to the wider multidisciplinary team (MDT), offering expert assessment, planning, delivery, and monitoring of care for patients with complex palliative needs. We are seeking a critical thinker with a wide range of clinical experience, strong leadership capabilities, and excellent communication and organisational skills.

Training & Progression:

The successful candidate will be appointed at Agenda for Change Band 7 while undertaking a Master's degree in Advanced Clinical Practice, funded via the apprenticeship route. Upon successful completion of the MSc and qualification as an ACP, the role will progress to Band 8a.



Assess patients and plan, implement and evaluate care using specialist clinical knowledge and skills Review patients and provide care in inpatient, outpatient, Virtual Ward setting and home visits Contribute to multidisciplinary approach to patient management Provide education and support to patients, carers and colleagues Participate in audit, research and service development within the Specialist Palliative Care service.

Perform independent specialist assessment of patients to formulate a diagnosis, plan and implement care and arrange ongoing follow up as an advanced practitioner using highly developed specialist clinical knowledge advanced clinical skills.



Independently manage a patient caseload providing expert specialist assessment, planning and evaluation.



To lead the multi-disciplinary team in resolving patient problems, by the provision of new and innovative models of care



Act as a role model and expert clinician providing in depth specialist clinical knowledge to colleagues through MDT meetings, for patients and carers / relatives.



Act as an innovative, enthusiastic role model providing leadership, guidance and advice to staff on operational and professional issues promoting an open and honest and transparent culture.



To improve the experience and outcomes of care for patients by taking the lead in investigations and treatments prescribed.



To provide a consistent and personalised service by ensuring that the patients receive the best care, in the best place from the best person or team.



To provide social, emotional and psychological support for patients from referral and throughout their treatment.

To deliver expert specialist evidence based care including:

development, assessment, planning, implementing, evaluating programmes of evidence based nursing care for a defined caseload.



To make appropriate referral to consultants/other specialist/members of the multidisciplinary team in primary, secondary or tertiary care settings.



To be aware of risk management issues both locally and nationally and develop strategies to address identified issues.
