Job Role Transitions Officer


Location Northern Health & Social Care Trust - Ballymena Office.


Salary Starting on 29,540 per annum, pro rata


Hours 18.5 hours per week





The Service





Transitions supports young disabled people explore options and next steps as a school leaver. The service offers specialist support to individuals so that they can make decisions about their future and personal development goals with confidence. The service supports young people and their families during this transitional period.




The Role





To deliver the Transition Service fully involving young people and their families to make decisions about their life after school. The Transition Officer will use an inter-agency approach to ensure a smooth transition post school into education, employment as well as exploring health & well-being opportunities in their local area.

Essential Criteria:





1.

Qualification:
RQF level 6 (e.g. Bachelor's Degree) or equivalent level professional qualification

AND

Minimum of 1 years' paid experience in the direct provision of education, training or support

OR

Minimum RQF 4 or equivalent in a relevant discipline

AND

Minimum of 3 years' experience as above

. Proficiency in IT skills, specifically Microsoft Word, Excel, Outlook

. Ability to work independently with good organisational, communication & interpersonal skills
