Job Description

We are recruiting to cover the established post-holder's role while they are on their 12 months maternity leave. It's an exciting role, jointly hosted by the Trust's Young Adults and Personality Disorder pathways, specialising in working with Yong Adults (18-25) who are making the transition from CAMHS to AMHS as they reach their 18th Birthday.



The transition of young people from child and adolescent services (CAMHS) to adult mental health services (AMHS) can be problematic . This role has been developed to assist with the assessment, risk management and care planning from the age of transition so that young people presenting with complex emotional needs are more ready and open to the way adult services work.



The post-holder will be responsible for championing the needs of young people (18 - 25) within the Camden and Islington Personality Disorder service. You will be required to work flexibly, independently and responsively within a dynamic working environment. Applicants need to be comfortable with outreach and multi-agency joint working and need to have experience in direct working with services users with personality disorder or enduring complex needs. Applicants need to be confident in providing case consultation to staff from a variety of professional backgrounds and have significant experience of multi-disciplinary team working.



The main focus of this role will be to provide assessment and therapeutic intervention to young people presenting with complex emotional needs, facilitating the smooth transition of young people from CAMHS to the adult personality disorder service. Alongside this, our work involves supporting the wider network to work more effectively with young adults who present with complex mental health problems, specifically 'personality disorder'. The post holder will provide support through:



Staff case consultation (either individually or in group session)

Liaison and navigation between different services in the care pathway

Training

Direct working with service users - including assessment and therapeutic case holding

In order to meet the needs of the Trust you may be required from time to time to work at different locations to your normal place of work. This may mean that you are required to work at any location that fall under Barnet, Camden, Enfield, Haringey or Islington. The Trust reserves the right to require staff to work at such other places or locations as it considers reasonable and necessary on a temporary or permanent basis.



To provide psychological assessments of young people using the PD service, using interview, formal psychometric and other assessment methods as appropriate. To conduct joint assessments of young people with the Core Teams when there are queries around personality related difficulties. To provide a training and consultation model for primary care management of young people presenting with PD To provide flexible practices within the PD service to facilitate the engagement of young people (18-25) with the service. To work closely with local CAMHS services to facilitate smooth transition of young people from CAMHS to adult services. To formulate and devise psychological treatment and management plans for young people with a range of problems of severity and complexity, including those with challenging behaviours, and to provide psychological treatment for such clients, using a range of psychological interventions appropriate to the service. To provide advice, guidance, and consultation to other professionals, to assist in the formulation, diagnosis and treatment of young people using the service. Opportunities for developing therapy offers to young people who are under the PD service.
