We are looking for an enthusiastic Practice Nurse to join our team. You will be supported by a team of clinicians and administrative staff including GP's, Clinical Pharmacist, Healthcare Assistants, PAs, FCPs and Care navigators. You will be required to:
Provide primary care nursing services including child immunisations, travel immunisations, cervical smears, wound management.
Chronic Disease Management (Desirable)
Asthma, COPD and CHD (Desirable)
Family Planning (Desirable)
Provide timely clinical and culturally appropriate health services (educating patients re lifestyle, using recall and reminder systems etc)
Attending de-briefs, clinical meetings as requested
Ensure all patient consultations are recorded accurately in the clinical notes. This includes records of both face to face and remote conversations.
Part-time 3 days per week - spread across the surgery opening hours which are 7:30am-7pm Monday-Friday
EA/IA, Saturdays and Bank Holidays as needed. Flexibility required.
