Treatment Team Practitioner Nursing Associate
Job Description
The role of the Nursing Associate is to work with the multi professional team and the Registered Nurse contributing to the ongoing assessment, providing, and monitoring care in line with the service pathways and as agreed with supervising clinician(s). They will monitor the condition and health needs of people within their care on a continual basis in partnership with the multi-professional team. The main role of the Nursing Associate will be participation in the treatment and physical health clinics across the borough footprint. In addition to being a part of a multidisciplinary team who administers anti-psychotic medication, offers the treatment of ECT and monitors the physical health of patients open to secondary mental health services.
The post holder will be based across multiple sites and GP Practices, promoting continuity of care and effective communication regarding the treatment and support for patients under the care of secondary mental health services.
