We are seeking to recruit dynamic Band 6 practitioners. The post is open to qualified Nurses, Social Workers and Occupational Therapists.

We are looking to recruit skilled and highly motivated practitioners to join us as Band 6 Urgent Care Practitioners in the CRHTT, working closely with the acute mental health hospital and community teams to support people in a mental health crisis. CRHT are able to offer all the benefits of a hospital admission whilst remaining at home, including home visits from our experienced multi- disciplinary team.. We are committed to a focused recovery model. This post involves 7 day week 365 days per year working on a rota basis.



You will be an excellent communicator, demonstrating empathy and facilitating an inclusive and collaborative environment and assessment to those service users accessing CRHT services with a particular focus on:



Utilising your excellent clinical skills and judgement to assess individual needs and risks



Demonstrate an understanding of clinical interventions relevant to the care group to include education, care plans and supportive discharge planning,



Work compassionately and creatively to identify therapeutic and evidence-based outcomes and care plans for people who are experiencing a crisis in their mental health



Work closely with carers, families and partner agencies to facilitate and promote the safety and the well-being of service users and their families/carers.



work collaboratively across the urgent care services being responsible for supportive discharge assessments and participating in multi- agency meetings.



We provide mental health and learning disability care for children, young people and adults across Sussex and for children and young people in Hampshire.

We are building a new hospital in Bexhill to replace our inpatient services at the Department of Psychiatry. This means that, if you are successful in your application, you will be transferred to the new facilities when it opens in 2026.



This is an exciting and dynamic role that offers the opportunity for development and implementation of high-quality services. Previous experience of working in acute services or home treatment teams is desirable, although not essential. We offer excellent staff support through clinical supervision and teamwork.



This service is integral to our Urgent Care Pathway and ensures people presenting to acute mental health services receive a compassionate, person- centred assessment and timely care plan.
