An exciting new opportunity has arisen for a motivated, innovative, enthusiastic, and experienced Clinical Nurse Specialist to join our Vascular speciality at Royal Glamorgan Hospital, within Cwm Taf Morgannwg University Health Board.
The post holder will have expert knowledge and will provide strong clinical professional leadership in their role as an advanced, autonomous practitioner providing patient-centred clinical care.
The Vascular Clinical Nurse Specialist will carry continuing responsibility for the assessment of care needs, development, implementation and evaluation of the programme of care, without supervision, for all vascular patients requiring all aspects of pre-, peri- and post-operative care. To focus on the individual needs of the patient and work closely with all disciplines to ensure promotion of high standards within the unit by using advanced knowledge and skills.

Main duties of the job
As a key member of the team, you will work closely with the Vascular Consultants/ Vascular Nurse, and Senior Nurse to ensure the delivery of high quality, efficient, safe and cost-effective patient centred care. The successful candidate will be expected to work within an outpatient setting, but occasionally review any patients across the surgical directorate, or patients that are outlying on Unscheduled Care wards. The candidate may also be required to work pan CTM, covering all hospital sites within the Health board. Due to the Vascular Network, with the base being University Hospital of Wales, there is an expectation that the candidate will spend time at the Vascular Unit in Cardiff, where there will be opportunity to partake in UHW ward cover and emergency assessment.
To succeed in this role, you will need to inspire and motivate a multidisciplinary team of staff, building excellent relationships through good communication skills. At this level the post holder is expected to work as a highly skilled autonomous practitioner managing their own work load and flexibly as part of the wider team. The individual must work across professional, organisational, agency and system boundaries to improve services and develop practice. They will participate in networks, locally, regionally and nationally.

Job responsibilities
To assist in the provision and to continue the development of a comprehensive service for vascular patients within Cwm Taf Morgannwg University Health Board. The Vascular Clinical Nurse Specialist (CNS) carries continuing responsibility for the assessment of care needs, development, implementation and evaluation of the programme of care, without supervision, for all vascular patients requiring all aspects of pre-, peri- and post-operative care. To focus on the individual needs of the patient and work closely with all disciplines to ensure promotion of high standards within the unit by using advanced knowledge and skills.
The Vascular CNS will work autonomously in Nurse-led Clinics and Pre-assessment providing patients and carers with specialist knowledge and perform clinical examinations and clerking, making clinical decisions on diagnoses that would normally be provided by medical staff. For example cancelation of patients operations if deemed not fit for surgery, commencing patients on best medical treatment, discontinuation of medication for surgical input and diagnosing peripheral vascular disease through history taking and vascular specific tests. The Vascular CNS will influence patient care through expert clinical practice, management of conditions, leadership, education to patients / carers and pre-and post-registered nurses and assisting junior doctors.
The Vascular Clinical Nurse Specialist will predominantly be based in an outpatient (OPD) setting and may be required to work across CTM sites performing Ankle Brachial Pressure Indices (ABPIs), Toe Pressures (TBIs), and complex dressings. There is also the expectation of hosting Pre- assessment clinics with an Anaesthetist on a regular basis.
This role will entail co-ordinating care for all vascular patients, being an integral part of the Multidisciplinary Team (MDT) and education of nursing and other health care professionals. The role will also involve receiving large Abdominal Aortic Aneurysm referrals who require surgery, from screeners in the community and managing the recently diagnosed aneurysm patients pathways throughout assessment, admission, discharge and follow-up.
The Vascular CNS is expected to participate in and promote clinical excellence and continuously develop the role as the service expands. This will be achieved through
Education and leadership
Continuous development of clinical skills
Research, audit and publication of papers, attending and presenting if required in monthly Vascular Audit Meetings.
Dissemination of experience through presentations at teaching sessions and Vascular Audit Meetings.
