Job Overview:


We have an exciting and rewarding opportunity available for an on-site experienced Complex Care Navigator to join our team at Broughton House Veteran Care Village in Salford Greater Manchester, engaging with the local veteran community.

Engaging with primarily with working age veterans with multiple complex needs you will provide information and support to ensure these veterans access the support they need from specialist providers across Greater Manchester.

The Armed Forces Covent Trust Fund has awarded us a grant to fund this full time post for the next 24 months.

Helping veterans and their families to navigate, often complex pathways of accessing statutory and volunteer health and welfare services, by providing information and support and a "holding hand" when necessary, to identify and engage with statutory and voluntary services within the community to meet these veterans multi-disciplinary/agency social care, health and welfare needs in a joined up manner.

Our Navigator will:

-Provide a single point of contact/expertise for veterans to understand and then access community-based support available to them, including providing expertise in providers' commitments to support the Armed Forces Community and their rights under the Community Care Act.

-Meet veterans face-to-face to undertake a needs-based assessment to understand the help they need and the challenges they currently face in accessing effective support in the community.

-Help the veteran develop a 'needs-led action plan', identifying areas where they need support and the outcomes they wish to achieve to improve their quality-of-life.

-Using their specialist knowledge of the health and social care system, help the veteran/carer navigate through the available support so they can effectively implement their action plan.

-Identify the most appropriate organisations to help the veteran with each aspect of their plan. E.g. benefits, debt, mental health, addictions, unemployment, isolation and or housing etc.

-The Navigator will build each persons confidence and self-advocacy capacity, accompanying them to initial meetings where needed and continuing to advocate for and support them until they have achieved their personal plan objectives

About You:



We are seeking a an individual with compassion and a flexible approach to welfare delivery, who is highly organised, analytical and an articulate communicator to provide advice and information on a 121 basis.

Our Navigator will:

Have an understanding and empathy with the veteran community.

Be tenacious in ensuring solutions and outcomes for those seeking our assistance.

Be able to correctly assess the the welfare needs of service users with unfaltering professionalism and sensitivity, providing a rapid solution driven personal response

Understand welfare benefit entitlements, and complete claim forms, escalating to specialist providers as needed. Be able to address drug, alcohol and gambling addictions & access mental health services. Find employment/training and housing where required.

Have a flair for building trusted relationships.

Have experience of making local contacts and networking with various services and providers to offer the widest range of opportunities to those we support .

Be non-judgmental approach and outstanding communication skills.

Have strong organisational and time management skills.

Capable of maintaining and developing strong multidisciplinary relationships but also able to provide an effective service delivery under their own supervision.
