As part of our on-going commitment to the delivery of high quality, reliable services for our patients, an exciting opportunity has arisen in the Primary Care Optometry Team within Hywel Dda University Health Board.
Main duties of the job
As a WGOS 4 Pathway Co-ordinator, you will support the transformation of eye care services across the Health Board.
The post holder will be responsible for maintaining records relating to the WGOS 4 pathway, including entering accurate data into the WPAS computerised system, whilst demonstrating the ability to handle patient information i.e. responsible for the administration and validation of clinical pathways. This will be carried out by liaising with clinicians and contractors in both Primary and Secondary Care, in a sensitive and confidential manner. Additionally, the post holder will be responsible for patient allocation in Primary Care, using clearly defined and agreed processes and the continued monitoring of the pathway.
The post will maintain effective relationships with stakeholders, using these to ensure the role supports the validation process surrounding WGOS 4. You will be expected to work closely with both clinical and managerial colleagues and provide service analysis, to ensure the delivery of high quality and appropriate services to patients.
