Key responsibilities include:

• Conducting comprehensive assessments to determine the needs of clients and developing personalized care plans.
• Collaborating with healthcare professionals, families, and community organizations to ensure seamless service delivery.
• Providing emotional support and guidance to clients and their families throughout the recovery process.
• Monitoring and reviewing client progress, adjusting care plans as necessary to ensure optimal outcomes.
• Maintaining accurate and up-to-date case records in compliance with legal and organizational standards.

The ideal candidate will possess the following essential qualifications and skills:

• A recognized Social Work qualification (e.g., Degree in Social Work, DipSW, or equivalent).
• Registration with Social Work England.
• Experience in adult social care, preferably within an intermediate care or rehabilitation setting.
• Strong communication and interpersonal skills, with the ability to build rapport with clients and their families.
• Excellent organizational skills and the ability to manage a diverse caseload effectively.
