Qualified Social Worker - Intermediate Care Services
Location: Walsall (Blakenhall Village Centre, WS3 1LZ)
Rate: 28.40 per hour
Contract: 1st November - 31st March
Working Pattern: Hybrid (minimum 2 days office-based)
Eden Brown Synergy is recruiting for an experienced Qualified Social Worker to join the Intermediate Care Services team within Adult Social Care at Walsall Council.
This is an exciting opportunity to work within a supportive multi-disciplinary service that focuses on promoting independence and supporting timely discharges from hospital or community reablement pathways.
Key Responsibilities:
Undertake Care Act assessments, reviews, and support planning to promote independence.
Work collaboratively with health professionals, reablement teams, and other agencies to facilitate safe and effective discharges.
Apply strength-based approaches to support individuals to regain or maintain their independence.
Complete Mental Capacity Assessments and safeguarding duties as required.
Maintain accurate records in line with local authority procedures.
Requirements:
Social Work qualification (Degree, MA, DipSW or equivalent).
Current registration with Social Work England.
Experience in adult social care, ideally within intermediate care, hospital discharge, or reablement services.
Ability to work effectively in a fast-paced, multi-agency environment.
Flexible approach to hybrid working - minimum of 2 days per week based at Blakenhall Village Centre (WS3 1LZ).
