Job Description
To have made an effective contribution to reaching the Trusts vision, strategic objectives and key work programs Evidence of consistent and sustained improvements in standards of practice and care in specialised area Evidence of staff development to meet patients' needs in specialised area of care. Evidence of contribution to the effectiveness and efficiency of care To participate in managing complex discharges and co-ordinate all relevant services to ensure timely, appropriate and safe discharges. To have made an effective positive impact on the patients experience whilst in hospital.
