We are looking for caring and reliable Weekend Care Workers to join our team in a residential home supporting adults with learning disabilities. This is a rewarding opportunity to make a real difference in people's lives while working flexible hours that suit you.

Responsibilities



Key Responsibilities:



1. Care Coordination:



To provide all aspects of personal care. Develop and maintain personalised care plans in collaboration with residents, families, and multidisciplinary teams. Monitor and review care plans to ensure they meet changing needs. Coordinate appointments, therapies, and interventions with healthcare providers, therapists, and other professionals. Facilitate smooth transitions for residents entering or leaving the care setting.

2. Advocacy and Support:



Advocate for residents' needs and preferences, ensuring their voices are heard and respected. Support residents in accessing appropriate services, including healthcare, education, and social opportunities. Help residents navigate complex systems, such as benefits, housing, and specialized services.

3. Family and Stakeholder Engagement:



Serve as the primary point of contact for families, providing regular updates and addressing concerns. Liaise with external professionals, such as social workers, GPs, and community support services. Organise and participate in care reviews and multidisciplinary meetings.

4. Promoting Independence and Well-being:



Identify opportunities for skill development and increased independence for residents. Encourage participation in social, recreational, and educational activities. Support residents in achieving personal goals and aspirations.

5. Compliance and Documentation:



Maintain accurate and up-to-date records of interactions, care plans, and progress notes. Ensure compliance with legal and regulatory standards, including safeguarding policies. Contribute to quality assurance processes and audits as required.

Requirements



Essential:



Experience working with adults with learning disabilities in a care or support setting. Strong understanding of person-centered care principles. Excellent communication and interpersonal skills. Ability to manage multiple priorities and adapt to changing needs. Knowledge of local resources, services, and benefits systems.

Desirable:



Relevant qualifications in health and social care (e.g., NVQ Level 3 or equivalent). Experience in a care navigator, care coordinator, or similar role. Knowledge of safeguarding and mental capacity legislation.

Empathy, patience, and a genuine passion for supporting individuals with learning disabilities. Strong organisational and problem-solving skills. Ability to work collaboratively within a team and independently. Commitment to promoting equality, diversity, and inclusion.
