A role supporting older people, improving health and wellbeing through enabling meaningful connection and practical support via signposting. You'll work within a vibrant, dynamic team at an award-winning charity. This will be a mixture of office and home-based working which will also involve visits to Link Friend homes. Use of a car essential.
OVERVIEW
The Wellbeing Officer will provide dedicated support to all of our Link Friends who have no clear next of kin or emergency contact, ensuring they are never left without help when needed. Additional support will be offered to those facing extra challenges, such as after a hospital stay, during bereavement, or following a significant change in circumstances that could lead to crisis. The role involves making welfare and feedback calls, carrying out home visits for re-assessment, and capturing the difference we make through short case studies, photos, and quotes. Working closely with staff and volunteers, you'll help ensure every Link Friend receives the right support at the right time.
CORE ASPECTS OF THE ROLE
? Make welfare calls and assessments to all Link Friends with no next of kin or emergency contacts. Oversee their wellbeing and ensure they have the correct support in place as they age alone.
? Provide support to those with extra needs (e.g. after hospital discharge, bereavement or change in circumstance).
? Carry out regular 'Keeping In Touch' and Client Call Feedback calls, documenting outcomes on our CRM.
? Conduct reassessments for existing Link Friends as required and as needs change.
? Conduct assessments for new referrals in times of high demand.
? Liaise with the assessment team and senior staff regarding any issues or concerns.
? Signpost to charity partners, organisations, and statutory services as needed (training provided).
? Work alongside the Safeguarding Officer to report and record safeguarding concerns.
? Maintain accurate records using our database.
? Liaise with family members, carers, and health professionals to achieve the best outcomes.
? Collect and update outcome information for monitoring and evaluation purposes.
? Write regular case studies/good news stories highlighting our impact for various publications and online content. This is required by Wokingham Borough Council and local parish councils.
PERSON SPECIFICATION
? Excellent interpersonal skills (verbal and written), able to relate effectively to older people, volunteers, and professionals.
? Ability to organise own work load.
? Strong understanding of confidentiality and professional boundaries.
? Ability to handle sensitive situations with empathy and discretion.
? Commitment to equality and diversity.
? Confident and proficient IT skills. (essential)
? Full driver's licence and use of a car.
