To work as part of a multi- disciplinary team on the Wellbeing unit, delivering high-quality, evidence-based holistic palliative care to patients and their families, ensuring all care is provided with compassion, dignity, and respect.

Undertake individual patient assessments, formulating goal setting and plans of care Be responsible for agreed service coordination and quality tasks, for example data collection, audits, attendance at multidisciplinary meetings. To lead creative therapies within the Wellbeing unit with support from volunteers and external services. Support and supervise the work of Support Workers, Students and Volunteers The post holder will provide individualised holistic enablement focused palliative rehabilitation within their scope of practice. Partake in the on-call rota. Contribute to and participate in organisational service development. Work across the clinical services, including Hospice at Home when required, to ensure service delivery.
