Wellbeing Support Advisor

to join our innovative

Care Connection Team

in Hillingdon. This is your chance to be part of a pioneering approach to integrated, person-cantered care.

What You'll Do:



As a Wellbeing Support Advisor, you'll be at the heart of a multidisciplinary team, working alongside GPs, Guided Care Matrons, and Health Care Support Workers. Your role will focus on:

Supporting adults (18+) with long-term conditions, social isolation, or complex needs. Delivering holistic, non-clinical interventions and social prescriptions. Managing a caseload through assessments, support planning, and referrals. Helping reduce unnecessary hospital visits by promoting community-based solutions. Advocating for clients and guiding them through health, social care, and voluntary services.

What You Bring

:

We're looking for someone who is:

Empathetic and people-focused

, with experience supporting individuals in health or social care settings.

Organised and self-motivated

, able to manage a caseload and work both independently and as part of a team.

Knowledgeable

, with an understanding of social prescribing, safeguarding, and the challenges faced by those with long-term conditions.

A great communicator

, confident in engaging with clients, professionals, and community partners.

A background in health or social care, motivational interviewing, or knowledge of the Hillingdon voluntary sector is a bonus--but not essential.
