Job Overview



Based within Solihull Integrated Addictions Service (SIAS) Young Persons team, the Young Persons Addictions Practitioner will offer support to young individuals who are misusing substances, or have other addictive behaviours.

This position involves working closely with clients to assess needs, co-develop tailored care plans, provide structured interventions and facilitate access to resources, opportunities, other services and activities. The ideal candidate will possess strong communication skills and a passion for making a positive impact in the lives of young people. They will work closely with families, schools, social services and other professionals to ensure a holistic approach to care.

The Young Persons Addictions Practitioner will work on an outreach basis, engaging clients in a variety of different settings, therefore, a Full UK Driving License and access to own car is essential. There is a need to work flexibly to meet client needs, so some evening and weekend work may be required.

Main Duties



Conduct assessments to evaluate the needs and circumstances of each client. Develop and implement individualised care plans for young clients struggling with addiction. Implement risk management and harm reduction plans. Provide one-to-one support to help clients navigate their recovery journey. Facilitate 'moving on' with positive lifestyle choices. Collaborate with other professionals to ensure comprehensive support for clients. Maintain accurate records of client progress and interventions in accordance with organisational policies. Contribute to reports required to evidence impact of work delivered. Utilise IT systems for documentation, reporting, and communication purposes. Engage in continuous professional development to stay informed about best practices in addiction support. Support colleagues in the SIAS YP team delivering family support, early intervention, prevention and engagement activities.

Qualifications, Skills and Experience



Proven experience in a similar role or within the field of youth work or addiction support. Hold, or have a commitment to completing, relevant qualifications to support your role. For example, Diploma/Certificate in Youth and Community work, DipSW, NVQ Level 3 Health and Social Care. Strong communication skills, both verbal and written, to effectively engage with young people and their families. Familiarity with developing care plans tailored to individual needs. Ability to respond appropriately to any safeguarding concerns. Proficiency in IT systems for record keeping and reporting purposes is required. Ability to work on own initiative but also contribute as part of a team. Willingness to work flexibly to meet the needs of the service.

Job Types: Full-time, Fixed term contract
Contract length: 12 months

Pay: 25,810.00 per year
