JOB TITLE: Activity Coordinator



RESPONSIBLE TO: Head of Activities & Home Manager

OVERALL OBJECTIVES



To work within the ethos of QHIP Ltd.

To ensure that all residents have access to and are enabled to participate in a range of activities which are designed to enhance their quality of life.

MAIN DUTIES



Provide an activity programme that is designed to meet the social, spiritual, intellectual, physical and emotional needs of residents. Ensure that the activity programme takes account of the residents' social and cultural diversity.

Ensure that all activities are appropriate to the age, needs, wishes and interests of the residents. Ensure that activities are reviewed to take account of changes detailed in individual care plans.

Carry out a social care assessment on each new resident to ensure that their individual needs, wishes, and interests are understood and catered for. Regularly review these assessments with individuals and / or their advocates.

Contribute to the care planning process by keeping up to date with all assessments and care plans, providing information for inclusion in the care plan and participating in reviews at the request of the Home Manager.

Ensure that residents are consulted on a regular basis about the range of activities on offer and encourage active participation in decisions about specific activities as far as possible.

Attend residents' meetings to encourage suggestions for activities and to share information about planned activities.

Organise activities for groups and individuals, ensuring that they are fully involved in making decisions about the choice of these activities and participating in them. Ensure that residents who choose not to socialise with others are not isolated, lonely or lacking in stimulation.

Maintain records of activities including residents who participated, staff who assisted and feedback received.

Carry out periodic surveys to measure satisfaction and outcomes. Liaise with the Home Manager to ensure compliance with the Activities Budget. Ensure all costings are agreed by the Home Manager before an activity is organised.

Maintain accurate financial records in respect of costs incurred in the provision of activities.

Display a list of activities on a weekly basis in visible locations throughout the Home, in a format the majority of residents can understand. Ensure that those residents who are unable to read notices are informed of the activities available. Give as much notice as possible to all residents if an activity has to be changed or cancelled.

Involve residents' families and friends in activities.

Accompany and support residents on trips outside of the Home e.g. shopping, visiting friends etc. Assist residents to manage their money and make purchases on their behalf if they ask you to do so (following Institute procedures at all times).

Organise events that promote the image of the Home as a member of the local community and encourage residents to be an active part of their community in appropriate settings.

Ensure that risk assessments are completed prior to organising activities that take place outside the Home, whilst respecting the right of the residents to take informed risks.

Promote effective communication by participating in staff meetings, handovers, and completing communication books, diary etc.

Encourage all staff to assist residents to participate in activities. Report to the Home Manager if staff are reluctant to get involved.

PERSONAL DEVELOPMENT



Take responsibility for ensuring that your skills and knowledge are up to date and attend relevant training courses, including mandatory training and other training specific to the post.

Attend and actively participate in supervisions and appraisals with your line manager.

Discuss any areas where additional support is required with the Home Manager.

GENERAL

Be familiar with and adhere to the companies policies and procedures in relation to your work to ensure that high standards of quality and good practice are maintained, developed and monitored. Be familiar with local and Institute procedures in respect of safeguarding and reporting allegations of abuse.

Ensure all reasonable precautions are taken to protect the safety and welfare of the residents, staff and visitors. Including following guidelines in respect of infection control. Report all incidents and potential hazards.

Ensure all people are treated with respect, compassion, hospitality, justice and trust in the course of your work, thereby promoting the company's core values and mission of equal opportunities through anti discriminatory practice.

Maintain confidentiality at all times. (Information concerning residents and staff, their conditions and affairs is strictly confidential and must not be disclosed to any unauthorised persons). Failure to maintain confidentiality could result in disciplinary action.

Carry out any additional duties as necessary to ensure the needs of the residents and the Home are met.

The post holder will be expected to work reasonable overtime or change shifts when required to meet the needs of the Home / residents. You will be required to work unsocial hours and weekends as necessary. The service operates 24 hours per day, seven day per week and flexibility is essential to meet the needs of the service.

This post is subject to checks carried out in accordance with the services Safeguarding Procedures including a DBS enhanced disclosure. The results of these checks must be deemed satisfactory by the Home Manager.

Job Types: Full-time, Part-time

Pay: 12.21-12.25 per hour
