JOB PURPOSE: To plan and implement activities appropriate to Patients' needs and requests. The post holder will work closely as part of the multidisciplinary team including Occupational therapists, Physiotherapists, Registered Nurses, care staff and volunteers.
MAIN RESPONSIBILITIES:
To promote and safeguard the welfare of vulnerable adults.
Help patients to socialise within the Community Rehabilitation Centre and provide a variety of activities that cater for all tastes.
Plan and initiate monthly rolling / individual programmes and encourage patients to maintain pre-existing hobbies.
Encourage Staff Members, Relatives and Friends to participate in the Centre's activities.
Accompany Patients, where possible, to participate in activities, which may occasionally take place outside normal working hours.
Help to create an atmosphere that suits individual Patients within the Centre.
Assist with fund raising, and budgeting, for entertainments, materials and outings.
Maintain full and accurate records of daily activities using appropriate documentation and assist the Named Carer to review and update patient care records.
Assist with administration including filing, photocopying and telephone answering. o Assist the Deputy/Registered Manager when interviewing relevant Volunteers and assistant Staff Members, and supervise their work, in line with the Company's policies and procedures.
Selling snacks/sweets, ancillary personal care items and shopping for materials for activities
Communication:
Discuss the aims and objectives of recreation therapy with other Staff Members.
Report any changes in Patients' physical or emotional condition to the Deputy/Registered manager or Person in Charge.
Provide comfort and company, on a one-to-one basis, for patients who are unable to undertake any form of activity.
Arrange / participate in Staff and patient meetings, as and when required.
To protect and maintain confidentiality of information concerning patients, visitors and staff at all times.
Act in a professional manner at all times when interacting with patients, relatives, colleges, other service users and visitors.
To feedback on patient's ability to join in with activities and techniques developed to settle the patient to support discharge planning.
Patient Care
Work with members of the multi-disciplinary team and volunteers to prepare and implement social, leisure and recreational activities with patients.
To devise and implement interesting programs of activity for people with a range of different disabilities and or medical conditions.
Provide 1-1 social contact with patients according to the individual ability of the patient to participate.
Ensure there are a range of individual activities for patients, e.g. board games, electronic games, music therapy, creative crafts and reading/observational material.
Organise social events on the wards to include patients, relatives, staff and visitors.
Organise and arrange individual/group activities for patients to promote social interaction.
Liaise with other outside agencies to provide support and advice on social activities.
Assist patients to maintain all aspects of daily living activities including toileting and personal care if required.o To maintain and improve mobility; contribute to movement and treatment to achieve physical comfort.o To maintain their personal hygiene and appearance. o Assist with eating and drinking and diet requirements. o Assist with access to toilet facilities. o Assist patients to maximize their independence. o Ensure safe practice to minimise the risk of infection to patients and staff in accordance with national and CHC policy.o Ensure and demonstrate written and electronic communication through up-to- date, succinct, clear, legible, accurate and comprehensive paper and electronic documentation.
Observe for and report any change in the condition of patients to XX.
Accurate recording and processing of patient information and patient care
Report all incidents and be familiar with DATIX reporting system.
Report all accidents and incidents to the Registered/Deputy Manager.
To provide personal care as designated in the personal plan and to promote independence and self-direction at all times whilst promoting dignity, privacy and showing respect.
To meet physical needs including toileting, washing, feeding and other personal care tasks.
Planning and Organisation
Work on own initiative making assessment of workload and adjustments to daily routine as required.
Assist patients in all activities in daily living when required.
Identifies and takes actions when own or others behaviours undermines equality and diversity.
Training & Development
Ensure all Staff Members know how to use appropriate equipment.
Attend mandatory training days/courses, on or off site, as and when required.
Maintain professional knowledge and competence.
Health & Safety:
Report immediately to the Deputy/Registered Manager, or Person in Charge, any illness of an infectious nature or accident incurred by a patient, colleague, self or another.
Understand and ensure the implementation of City Health Care Limited's Health and Safety policy, and Emergency and Fire procedures.
Report to the Deputy/Registered Manager any faulty appliances, damaged furniture, equipment or any potential hazard.
Promote safe working practice in the Centre.
Keeps clinical, patient areas and staff areas safe, clean and tidy.
General:
Promote a positive personal / professional profile within the local community, ensuring the good reputation of City Health Care Limited at all times.
Ensure that all information of confidential nature gained in the course of duty is not divulged to third parties.
Ensure the security of the Centre is maintained at all times.
Adhere to all Company policies and procedures within the defined timescales.
Ensure all equipment is clean and well maintained.
Carry out any other tasks that may be reasonably assigned to you.
