The main purpose of an Activity Co-ordinator / Wellbeing Co-ordinator is to lead and enable people living in our care homes to participate in recreational and meaningful activities, as well as collaborating with our local communities and partners. This includes acknowledging the persons individual capabilities, as well as their interests and preferences.
Reports to:
Home Manager.
Responsible to:
Operations Team.
Main Roles & Responsibilities:
Communicate with each individual person in order to identify a person's social and recreational preferences and needs.
Create a personalised activity / wellbeing care plan for each person, review and update this care plan monthly (minimum).
Evidence and develop positive outcomes.
Build relationships with the Local communities and Partners.
Acknowledge an individual's Personhood (the status of being a person i.e. 'seeing the person'), plus embedding a culture of person centredness (this approach treats each person respectfully, as an individual human being and not just a condition to be treated).
Recognise a person's spirituality (this is a person's beliefs / faith, as well as recognising a person's sense of wellbeing i.e. having a sense of peace and purpose).
Enable a person to exercise their 'Human Rights' and 'Choices', whilst maintaining their respect, privacy and dignity
Show kindness and compassion towards others.
Refer to the 'Health and Social Care Standards' (Care Inspectorate, 2017).
All staff members should maintain their 'Personal Development'.
All staff to adhere to Enhance 'Policies and Procedures'.
Communication:
Attend team meetings and share outcomes and ideas with those in attendance.
Liaise with the relevant persons and discuss 'What is working well', as well as 'What we could do better'. This information should be reflected within the person's care plan.
Acknowledge 'Relationship centred care' (sense of belonging and purpose) - maintaining established relationships and routines developed prior to admission.
Effective communication is essential to achieving success.
Collaboration & Teamwork:
When supporting individuals, it is fundamental that we Identify 'what and who' is important to the person.
Liaise and involve friends and families where appropriate.
Contact local communities and work collaboratively with others to achieve positive outcomes eg involve local school, cafes, churches and 'working groups.'
Maintaining good 'Teamwork' is essential - teamwork is a core value which signifies the importance of collaboration, mutual respect and collective responsibility in achieving common objectives.
Refer to good practice guidance and recommended tools to gain positive experiences for people.
Supervision & Appraisal:
Attend regular supervision with line manager, as well as participating in an annual appraisal.
Learning & Development:
Update EVOLVE statutory and mandatory E-learning in line with Enhance protocol.
Attend recommended training identified by the Home Manager,
Each staff member has a responsibility to maintain their own personal development.
Confidentiality:
All staff MUST respect confidentiality, as well as confidential information about a person. Staff MUST gain consent before sharing, this includes adhering to the principles of GDPR.
Consent MUST be gained prior to taking photographs and/or videos of a person, especially when sharing on social media.
Recognising abuse:
All staff to ensure that people living in our care homes are protected from abuse and free from harm and neglect.
Where abuse or harm is considered / observed, you have a responsibility to notify the person in charge immediately. Failing this, inform the Deputy Manager and subsequently the Home Manager where necessary.
Where requested, you might need to write a statement of account of events and/or a reflection of your observations.
