About the role

Libertus Services provide day care placements, Community Outreach and online activities for older adults in the South East and South West of Edinburgh at our Gracemount service. We provide activity groups, meals and community activities to enhance the lives of older people, and maximise their potential to be as independent as possible.

We require a Part Time Support Worker to join our team across our services in the South East of Edinburgh, providing opportunities and activities that promote the best possible outcome for the people we support. Duties include leading group activities, supporting people to travel on our minibus and providing some personal care to members of Libertus in accordance with individual support plans.

Responsibilities

To identify and monitor the needs and interests of all users through client profiles and support plans within Key Worker responsibilities

Monitor, evaluate, support and implement individual outcome focused action plans using Outcome STAR

Support our members with a positive and person centered approach to reflect Health and Social Care Standards and ensure the highest quality of care

Plan, prepare and lead a range of leisure opportunities during Centre based activity and Community based services

Co-ordinate the provision of activities in conjunction with colleagues and volunteers.

What we need from you

Commitment, enthusiasm and a positive attitude

Work well as part of a team, and as a lone worker

Good understanding of both written and spoken English

At least 2 years Social care experience in a similar role (Essential)

Min SVQ 2 Health and Social Care (Preferred)

Hours of work : 22 hours over 3 days, Monday, Wednesday and Friday Weekdays only, no evening or weekend shifts Annual leave entitlement, plus public holidays (Pro Rata)

PVG check will be sought through Disclosure Scotland as part of Libertus Services employment checks. A personal Interview will also be required as part of the employment process.
