We are seeking a dedicated and compassionate Qualified Social Worker to join our Adults Learning Disabilities Team in Cardiff. This is an exciting opportunity to work within a dynamic team, supporting adults with learning disabilities to lead fulfilling lives. The successful candidate will play a crucial role in assessing needs, developing care plans, and promoting independence among service users.
Key responsibilities include:

• Conducting comprehensive assessments of individuals with learning disabilities to identify their needs and preferences.
• Developing, implementing, and reviewing person-centered care plans in collaboration with service users, families, and other professionals.
• Providing support and guidance to individuals and their families, ensuring they are informed of available resources and services.
• Working closely with multidisciplinary teams to ensure holistic care and effective service delivery.
• Maintaining accurate and up-to-date records in compliance with local and national policies.

To be considered for this role, candidates must possess the following essential qualifications and skills:

• A recognized social work qualification (e.g., Degree in Social Work, CQSW, CSS).
• Registration with Social Care Wales.
• Experience working with adults with learning disabilities, preferably in a social work capacity.
• Strong communication and interpersonal skills, with the ability to build rapport with service users and their families.
• Knowledge of relevant legislation, policies, and best practices in social work.
