What You'll Be Doing
Conducting detailed clinical assessments to support timely, safe decision-making.
Prescribing and administering medications in line with clinical guidelines, while monitoring for effectiveness and safety.
Creating and coordinating personalised care plans with a multidisciplinary team.
Performing diagnostic procedures and interpreting results to inform treatment.
Educating patients and their families in managing long-term conditions and promoting preventative health.
Keeping accurate, compliant clinical documentation.
Contributing to audits, quality improvement, and patient safety initiatives.
Engaging in CPD and supporting innovation in clinical practice.

What We're Looking For
Experienced ACP with a background in primary care or urgent care.
Confident in patient assessment, prescribing, and chronic disease management.
Strong clinical knowledge, including anatomy, physiology, and pharmacology.
V300 independent and supplemental prescribing
Committed to high standards of care, reflective practice, and teamwork.
Skilled communicator who thrives in a collaborative, multidisciplinary environment.
