. This role is ideal for highly skilled clinicians who can work autonomously, assess undifferentiated patients in their own homes, and deliver high-quality urgent and routine care across the community.
You will work as part of a dynamic multidisciplinary team providing safe, efficient, and patient-centred visiting services to support GP practices, PCNs, and local urgent care pathways.
A Master's degree or Level 7 Advanced Practice modules and strong urgent care experience are essential.
Key Responsibilities
Provide autonomous assessment, diagnosis, treatment, and management of patients in their own homes.
Visit patients requiring urgent same-day assessment (worsening chronic illness, minor illness, acute presentations).
Undertake advanced clinical examinations and diagnostic decision-making.
Manage acute conditions, minor injuries, infections, exacerbations of long-term conditions and frailty-related presentations.
Safely prescribe medications (V300 desirable or willingness to work toward).
Liaise with GP practices, MDT teams, urgent care services, and community providers.
Escalate or refer patients appropriately to secondary care, community care, or specialist teams.
Maintain accurate, timely documentation using digital clinical systems.
Support safe service delivery through excellent communication, clinical judgement, and prioritisation skills.
Essential Requirements
Registered Nurse / Paramedic / AHP (NMC or HCPC).
Master's Degree in Advanced Clinical Practice OR significant Level 7 modules (Advanced Assessment, Clinical Decision-Making etc.).
Proven experience in Urgent Care, Home Visiting, Primary Care, UTC/MIU, or Emergency Care.
Ability to work autonomously with undifferentiated patients.
Strong advanced assessment and diagnostic reasoning skills.
Excellent communication, clinical judgement, and safety awareness.
Desirable Requirements
V300 Independent Prescribing Qualification.
Experience working within GP, PCN, or community urgent care models.
Previous experience supporting frailty pathways or community response teams.
Ability to work flexibly across weekdays and weekends if required.
