Overview:


The Advanced Clinical Practitioner (ACP) works autonomously within the general practice team to provide high-quality, patient-centred clinical care. The role combines advanced clinical assessment, diagnostics, treatment, and decision-making, supporting improved access and continuity of care within primary care.

Key Responsibilities



Provide

comprehensive clinical assessment

, diagnosis, and management of patients with undifferentiated and undiagnosed conditions. Undertake

autonomous consultations

(face-to-face, telephone, online) and develop treatment plans, including prescribing where qualified. Order, interpret, and act on

diagnostic investigations

(e.g., blood tests, imaging, ECGs). Manage patients with

acute, chronic, and complex health conditions

, including long-term condition reviews. Deliver

urgent care

within the practice, triaging and managing same-day presentations. Work collaboratively with GPs, nurses, pharmacists, and the wider MDT to ensure safe, effective patient care. Participate in

clinical governance

, audits, quality improvement initiatives, and evidence-based practice. Maintain accurate and timely clinical records in line with practice and regulatory standards. Provide

health promotion

, self-management support, and patient education. Contribute to practice development, service redesign, and workforce sustainability.

Qualifications & Requirements



Registered healthcare professional (e.g., Nurse, Paramedic, Pharmacist, AHP) with current professional registration. Completion of a

Master's level ACP programme

or equivalent training aligned to the four pillars of advanced practice.

Independent Prescriber

Evidence of ongoing

professional development

and competence.
