What You'll Be Doing



As an ACP at Lombard Medical Centre, you will be delivering high-quality, person-centred care across a blended role comprising both clinic-based consultations and home visits. You'll work autonomously, drawing on your advanced clinical assessment and diagnostic skills, and collaborate closely with our experienced multi-professional team.

Your core responsibilities will include:

- Assessing, diagnosing, planning, treating and reviewing patients with both acute and chronic conditions

- Carrying out home visits and leading care for patients with learning disabilities

- Managing clinical episodes independently including prescribing and referrals

- Taking a lead in unscheduled and emergency care

- Performing clinical procedures and planning care pathways

- Communicating effectively with patients, carers and clinical teams

What You Bring



We're looking for compassionate, capable clinicians who want to make a genuine impact in general practice. You'll need:

- MSc in Advanced Clinical Practice (or equivalent Level 7 qualification)

- Independent prescribing qualification

- Confidence working autonomously

- Strong communication and decision-making skills

- Passion for improving patient outcomes and mentoring others

- 9 GP Partners

- 3 Salaried GPs - ?if we add 5 from September

- GP registrars

- 2 Clinical Pharmacists

- First Contact Physio

- Long-Term Conditions Nurse

- 4 Practice Nurses

- 2 Nurse Associates

- 3 HCAs

- 1 Phlebotomist

The Details



Hours: 37.5 hours/week (full time) or part-time
Salary: Agenda for Change Band 8a
Location: Lombard Medical Centre, Nottingham
Accountable to: Clinical Lead/Practice Manager
Start date: Negotiable
