We are seeking a dynamic, resilient Advanced Clinical Practitioner (ACP) to join our growing multidisciplinary team, consisting of GPs, ACPs, Clinical Pharmacists, Specialist Chronic Disease Nurses, Paramedics, Mental Health Practitioners, and HCAs, alongside a strong and experienced management and admin team that is committed to delivering excellent care to our patients.

The role:
The post holder will serve as an Advanced Clinical Practitioner (ACP) at Maritime Health Partnership (MHP), delivering high-quality, patient-centred care to frail and vulnerable individuals in both care homes and the community. The role involves working autonomously and as part of a multidisciplinary team, managing patients with complex needs such as frailty, multi morbidity, and housebound patients.
You will conduct comprehensive assessments, make accurate diagnoses, and deliver appropriate treatments while developing and implementing personalised care plans. The ACP will be responsible for providing both proactive and reactive care, ensuring timely interventions for patients' long-term and urgent care needs.
The post holder will collaborate with internal and external healthcare professionals, including GPs, community nursing staff, palliative care teams, social workers, and other multidisciplinary team members, to deliver coordinated care. The role will also involve leading advanced care planning discussions, providing expert end-of-life and palliative care, and ensuring safe and effective care for patients.
Additionally, you will mentor colleagues, contribute to service development, and participate in quality improvement initiatives to maintain the highest standard of care across MHP and the wider practice.
Qualifications Required:
RGN/GPhC/HCPC registration
MSc in Advanced Clinical Practice or an equivalent MSc, or NHSE Advanced Practice Digital Badge (or working towards it)
Independent prescribing qualifications and current clinical practice in an advanced practice role
Key Responsibilities:
Provide a high standard of patient care to frail and vulnerable patients across various settings (care homes, patient homes, surgeries, virtually, and by telephone).
Manage a clinical caseload for patients with complex needs, including proactive and reactive care.
Lead and participate in advanced care planning discussions, developing personalised care plans.
Provide end-of-life and palliative care, including completing/amending Respect forms and facilitating discussions with patients and families about their wishes.
Collaborate with a multidisciplinary team, including care home staff, palliative care nurses, community nurses, and other specialists.
Deliver care in response to urgent care needs and support other ANPs in urgent care provision.
Mentor and support healthcare professionals to maintain clinical skills and knowledge.
Take an active role in service development and the enhancement of frail patient care.
The ideal candidate:
Strong experience working in primary care, community care, or out-of-hours services.
Comfortable working autonomously in a variety of clinical settings.
Excellent ICT skills and strong interpersonal and communication skills for dealing with a diverse patient population.
Experience in managing patients with long-term conditions and general nursing interventions.
Ability to contribute to administrative and professional responsibilities, ensuring accurate record-keeping of consultations and treatments.
Note:
Successful candidates are required to provide documented evidence of Full Immunisation History as a condition of deployment. This is a standard requirement for individuals working in a Care Quality Commission (CQC) regulated health or social care workplace unless exempt.
