About the Role
Remote Clinical Providers are seeking an
Advanced Clinical Practitioner (ACP)
to join a supportive GP practice team in the Stratford area. This role is offered on a
contractor basis
through RCP, with flexible and competitive rates.
You will be providing advanced clinical care within the practice, contributing to patient management, triage, and long-term condition support. You will work both independently and collaboratively as part of the wider multidisciplinary team, with a strong emphasis on same-day access and high-quality patient care.
Key Responsibilities
Assess and triage patients, including same-day triage.
Provide definitive treatment where appropriate (including prescribing medications in line with PGDs, NICE/local guidelines, and care pathways).
Manage undifferentiated and undiagnosed presentations, identify red flags and serious pathology, and take appropriate action.
Use complex decision-making to inform diagnosis, investigations, and the full management of episodes of care.
Deliver person-centred and population-based care, enabling shared decision-making.
Contribute to quality improvement, education, and CPD supervision for other roles within primary care (e.g., first contact practitioners, personalised care roles).
Requirements
Applicants must:
Hold a
Master's degree (or equivalent) in Advanced Clinical Practice
or a relevant clinical area.
Be working at a
Master's level across the four pillars
: clinical practice, leadership/management, education, and research.
Demonstrate core ACP capabilities and relevant area-specific clinical competencies.
(For advanced practice pharmacists only): have completed an approved 18-month training pathway (or equivalent) and be a qualified
Independent Prescriber
.
Be registered with the appropriate professional body (NMC / HCPC / GPhC).
