This is a key position within our service, offering the chance to work at an advanced level of practice and contribute to the development of high-quality, patient-centred care within the community.
The successful applicant will be working towards, or already hold, a
Master's qualification in Advanced Professional/Clinical Practice
, and will bring with them a strong background of senior clinical experience within cardiology. You will be confident in complex decision-making, able to demonstrate advanced assessment and diagnostic skills, and committed to delivering the very best outcomes for patients.

Main duties of the job
The post holder will be responsible for working clinically autonomously as an Advanced Clinical practitioner within the Countywide Cardiac Nurse Team and linked to Admission Avoidance services to provide senior clinical support for patient centred care to patients eligible for the Cardiac Nurse service.
The post holder will be based in the Countywide Cardiac Nurse Team and working with the Virtual Ward as part of the Cornwall @ Home Team.
Cornwall @ Home Services in a developing service which includes the Virtual Ward and the post holder will have a key role in developing services particularly to their previous experience for example heart failure pathway or management of delirium.
The post holder will be highly competent, visible and experienced autonomous practitioner who uses specialist knowledge and advanced skills to support the new way of working that emphasises a more efficient and patient focused service. This will ensure the safe referral or discharge of patients with heart failure undifferentiated and undiagnosed presentations in any area of the Urgent Treatment Centre.
The post holder will be able to take a relevant history; perform a holistic physical and mental health examination; undertake clinical assessment to establish a management plan and evaluate the progress of that plan within the limitations of their scope of practice. This will be carried out under the non-direct supervision of a medical mentor.
