Role Summary
We are seeking a skilled and dedicated Locum Advanced Clinical Practitioner (ACP) to provide high-quality care in a dynamic primary care setting. The role involves autonomous management of a wide range of acute and chronic conditions, working collaboratively with GPs and healthcare professionals to deliver exceptional patient care. Key Responsibilities
Conduct autonomous consultations for untriaged patients, managing minor ailments, acute conditions, and long-term condition reviews (e.g., diabetes, asthma, hypertension).
Prescribe medications independently in accordance with clinical guidelines and local protocols.
Perform clinical assessments, including history-taking, physical examinations, and diagnostic test interpretation.
Utilize
EMIS
software for patient record management, referrals, and prescription processing.
Support triage processes, prioritizing patient needs and coordinating with clinical staff for seamless care delivery.
Collaborate with multidisciplinary teams to ensure holistic patient care and continuity.
Participate in health promotion and chronic disease management clinics.
Adhere to GDPR, safeguarding, and infection control protocols to maintain patient safety and confidentiality.
Contribute to practice meetings and quality improvement initiatives as needed.
Person Specification
Essential Qualifications and Experience
MSc in Advanced Clinical Practice or equivalent postgraduate certification.
Registered with a relevant professional body (e.g., NMC for nurses, HCPC for paramedics).
Independent prescribing qualification.
Proven experience in primary care or urgent care settings, with competence in managing acute and chronic conditions.
Proficiency in
EMIS
and clinical systems.
Strong clinical decision-making and autonomous working skills.
Desirable
Experience in Somerset primary care settings.
Training in minor illness management or chronic disease care.
Familiarity with local referral pathways and NHS guidelines.
Skills and Attributes
