Job Description

Practice autonomously as an Advanced Clinical Practitioner within Primary Care, working within agreed clinical scope of practice and local protocols. Manage patients presenting with a range of acute and chronic medical conditions, providing safe, evidence-based clinical assessment and advice. Undertake comprehensive history taking, clinical assessment, examination, diagnosis, and treatment planning for patients presenting with a variety of conditions. Act as an Independent Prescriber (V300), prescribing safely and appropriately within areas of clinical competence.

Assess and manage all patients presenting with minor illness, in line with practice policy and clinical guidelines. Work collaboratively with GPs and the wider multidisciplinary team to ensure coordinated, high-quality patient care. Take an active role in clinical supervision, mentorship, and reflective practice to support ongoing professional development. Deliver chronic disease management clinics as required, in line with national and local guidelines.

Refer patients to secondary care services appropriately, following agreed referral pathways. Maintain accurate, timely, and comprehensive clinical records, including the use of SNOMED coding.
