Hartington Surgery are seeking a dedicated and skilled Advanced Clinical Practitioner to join our Small but busy, rural practice. The successful candidate will play a pivotal role in delivering high-quality patient care, utilising advanced clinical skills and knowledge. This position requires a strong foundation in anatomy, medication administration, and a commitment to improving patient outcomes.
Responsibilities
Provide comprehensive patient assessments and develop individualised care plans.
Collaborate with multidisciplinary teams to ensure coordinated patient care.
Educate patients and their families on health management and treatment options.
Maintain accurate and up-to-date patient records in compliance with regulatory standards.
Conduct follow-up consultations to evaluate treatment effectiveness and make necessary adjustments.
Stay informed about advancements in clinical practices and contribute to continuous improvement initiatives.
Undertake home visits as required.
Qualifications
Master's Degree (or working towards) or equivalent qualification.
Proven experience in patient care within a clinical setting.
Non-Medical Prescriber.
Excellent communication skills, both verbal and written, with the ability to engage effectively with patients and colleagues.
Strong organisational skills with the ability to manage multiple priorities in a fast-paced environment.
A commitment to ongoing professional development and adherence to best practices in clinical care.
